(signed by both parties)

B. Additional documents:

I. For individual investors participated in BCC, supplement:

Authenticated copy of Passport and/or Vietnamese ID card of investors (original)

Certified translation of Bank Statement (original)

II. For Corporate Entity participated in BCC, supplement:

Certified translation of consular legalized of Incorporation Certificate (original)

Certified translation of legalized copy of 2 latest years audited financial report (original)
(audited of the latest year)

Minutes of Meeting on participarting in BCC to carry out the Project (original)
(of all members of Board Directors)

Decision on participarting in BBC to carry out the project (original)
(of Chairman of Board Directors)

Power of Attorney (1) (original)
(issued by investors for the authorized representative of the Corporate Entity)

Authenticated copy of Passport or ID cards of authorized representative of Corporate Entity

III. In case investors need to lease factory or office to carry out the BCC, supplement:

Factory/ Office MOU Lease Contract

Certificate of Land Use Right and property attached to the land (an authentic copy)
(to prove that the landlord is the true owner of the factory rented by investors. The owner of the factory has responsibility to provide this document)

Business registration Certificate (an authentic copy)
(to prove that the lessor has factory/office leasing function)

IV. For the project which has investment capital of over 300 billion VND, supplement:

Economic and Technical explanatory statement (original)
(including some main contents: objectives, scale, location of investment, invested capital, schedule for implementation of the project, land use requirements, and technological and environmental solutions)

V. For case of authorization to submit application file:

Power of Attorney (1) (original)
given by investors to the person who submits application files.

Additional information

* For Passport or ID card of investor/legal representative of the Company to be established/authorised person: Investor can submit either translation or authentication version of those documents.

* Within 3 working days after the application dossiers are submitted, investors can contact Hai Duong's Department of Planning and Investment to verify the status of the application dossiers.

All foreign documents need to be translated into Vietnamese and authenticated or consular legalized as required by Laws.
